Targeted Case Manager

Our agency is seeking a full time experienced Child/Adult Targeted Case Manager to coordinate efforts to ensure that persons served receive holistic service planning, continuity of care, effective services and on-going advocacy in accordance with Medicaid documentation requirements and agency policy and procedures. Targeted Case Managers coordinate the development of individualized, comprehensive service plans, which allows persons served the opportunity to achieve their goals and optimize their level of independence. Targeted Case Managers act as advocates for persons served; they link persons served with needed services; monitor persons served progress and provide individualized supports. Targeted Case Management services and activities should be provided in community locations ,which may include: persons served residences, medical and behavioral health sites, hospitals, community resource sites, and shelters.

 

Responsibilities:

 

Intakes, assessments, service plans. Provides resources, home visits, referrals, education, prevention and intervention services.

Assures adequate and complete documentation of service plans, progress notes, billing, and all other required documentation for assigned persons served.

Coordinates needed ancillary services with other staff, physicians, agencies and facilities. Attends various meetings, staffing, workshops.

Ensures follow-up of all persons served.

Maintain expected level of productivity.

 

Education/Experience Requirements:

 

A targeted case manager for individuals with Severe Mental Illness, Serious Emotional Disturbance, or Substance Use Disorder shall:

 

Possess a Bachelor of Arts or science degree in a behavioral science; or

Be a certified alcohol and drug counselor who has a Bachelor of Arts or science degree; or

Have provided targeted case management services to a recipient any time from April 1, 2014, to the effective date of this administrative regulation; or

Have supervised the provision of targeted case management services to a recipient any time from April 1, 2014, to the effective date of this administrative regulation; and

Have at least one (1) year of full-time employment experience after completing the educational requirements:

 

Working directly with adults in a human service setting; or

Working directly with individuals under the age of twenty-one (21) in a human service setting;

Successfully complete a department approved targeted case management training within six (6) months of employment as a targeted case manager; and

Successfully complete department approved continuing education requirements every three (3) years thereafter.OR

A master’s degree in a behavioral science (see below) may substitute for the one (1) year of full-time employment experience.

 

Successfully complete a department approved targeted case management training within six (6) months of employment as a targeted case manager; and

 

Successfully complete department approved continuing education requirements every three (3) years thereafter. AND

 

Must have a valid Kentucky Driver's License and a safe driving record.

 

 

"Behavioral science" means:

(a) Psychology; (b) Sociology; (c) Social work; (d) Family studies; (e) Human services; (f) Counseling; (g) Nursing; (h) Behavioral analysis;  (i) Public health; (j) Special education; (k) Gerontology; (l) Recreational therapy; (m) Education; (n) Occupational therapy; (o) Physical therapy; (p) Speech-language pathology; (q) Rehabilitation counseling; or (r) Faith-based education.
